How to Fill Out Your Work Authorization
Hi, this is My Water Damage Hero. This short video walks you through filling out our Work Authorization form, step by step. It only takes a couple of minutes, and every field matters, so let's go through it together.
We Can't Start Without Your Signature
First, the most important thing: we cannot start any emergency work until this form is signed. Your signature is what gives us the legal go-ahead to begin, so please complete it as soon as you can.
Project Number & Date
At the top of the form, enter your project number. It starts with the letters M, W, H, D, and you'll find it in the email we sent you. Then add today's date.
Your Name, Phone, Address & Email
Next, your information. Your full name, because you're listed as the Owner on the contract. Your phone number. The property address, meaning the home where the damage is and where we'll be working. And your email address, so we can send you a copy of the signed form.
Insurance Carrier & Claim Number
Now the insurance section. Write the name of your insurance carrier, and your policy or claim number if you have one. If you haven't filed your claim yet, that's okay. Those two boxes can be left blank for now.
This Form Lets Us Talk to Your Insurance
Here's why this form matters for your claim: signing it gives us permission to talk to your insurance company and your adjuster on your behalf. Without your signature, we legally cannot discuss your claim with them at all.
Anticipated Start Date
Next, the anticipated start date. This is simply the date your project is estimated to start, the date we discussed with you. It's our planned first day of work.
Your Deductible Is Due at Time of Service
About payment: your insurance deductible is due at the time of service. Please plan to pay your deductible when the work begins.
Read the Contract & Check Both Boxes
Then please read through the Emergency Services Contract shown on the form. When you've read it, check both boxes, to confirm you've read the contract and the emergency authorization.
No Handwritten Changes
One more rule: please don't write changes by hand on the form. Handwritten changes are not effective. If anything about the work needs to change, we handle it with a change order, signed by both you and us, so everything is agreed in writing.
Sign, Print Your Name & Date It
Finally, sign your name in the signature box, print your name underneath, and add the date you signed. On the online form, you can draw your signature right in the box, with your finger or your mouse.
That's It — Hit Submit
That's it. Hit submit, and we'll take it from there. If you have any questions while you fill it out, call or text us at six one zero, seven zero eight, two one nine three. Thank you for trusting My Water Damage Hero.
Frequently Asked Questions
What do I need before I start the work authorization form?
Your project number (it looks like MWHD-#### — our team gives it to you), your contact details, the service address, and, if you are filing an insurance claim, your carrier name and claim number.
When does emergency work start?
Work is scheduled once your form is signed. No emergency services begin until the work authorization is signed.
When is my deductible due?
Your deductible is due at the time of service. If we invoice your insurance carrier directly and the carrier pays less than the invoice, the remaining balance is due within 30 days.
What if I don't have my project number?
Call us at (610) 228-7440 or text Jessica at (610) 708-2193 and we will give it to you — the form cannot be matched to your project without it.
Can I sign on paper instead of online?
Yes. The same Emergency Services Contract is included in the insurance claim booklet we send every claim client, and our team can bring a paper copy. The online form is simply the fastest way to get your project scheduled.
